Lifestyle Health Coaching as Part of Your Care Team
A Lifestyle Health Coach is a valuable member of the Interdisciplinary Team (IDT), working alongside physicians, nurses, therapists, care managers, social workers, and caregivers to support whole-person health.
We help bridge the gap between medical appointments and everyday life. Using the principles of Lifestyle Medicine and Food Is Medicine, we help seniors build sustainable habits that support healthy aging, independence, and quality of life.
How We Support You
- Turn your physician's recommendations into realistic daily habits using SMART goals.
- Stay engaged and accountable to your care plan
- Track meaningful health goals and celebrate progress
- Organize medication lists, provider contacts, and specialist information
- Prepare for appointments with questions and health updates
- Maintain communication with family members and caregivers (with your permission)
Our focus is to empower seniors to remain active, informed, and independent while complementing the care provided by their medical team.
Care Team Collaboration
Lifestyle Health Coaching fits seamlessly within the Program of All-Inclusive Care for the Elderly (PACE) model. Working alongside your dedicated physician and Interdisciplinary Team, we provide ongoing support between appointments to help you successfully manage your health at home.
PACE services are funded by Medicare and Medicaid for eligible participants. Private-pay coaching is also available for seniors who are not covered through Medi-Cal or Medicaid.

This coaching service includes:
6-month program

12 sessions total | Two, 30-minute virtual calls or video conferencing appointments monthly

Self-monitoring device awareness using HIPAA-compliant devices and platforms.
Choose one connected device based on your goals and/or your physician's suggestion:
• Blood pressure trends | Blood Pressure Monitor• Glucose tracking | Blood Glucose Monitor* & CGM• Sleep patterns• Weight and recovery trends | Digital Weight Scale• Activity and hydration awareness• HRV, Oximeter*Blood glucose/CGM monitoring recommendations should be discussed with your healthcare provider when appropriate.

1. 20-Minute Welcome & Technology Setup
We'll help you integrate the wearable health technology for home, self-monitoring health metrics, connect your device, navigate the app and establish the cadence for future calls.

2. Two 30-Minute Coaching Sessions Each Month
Personalized coaching focused on intention, lifestyle medicine, and goal setting. Receive curated resources, recipes, and wellness tools to support you.
3. Monthly Progress Summary
Receive a personalized summary highlighting trends, and data to support your ongoing wellness journey and adjustments as your goals evolve. This summary is an important communication tool to bring along to doctor's and specialist's appointments
